NOTICE: WPI-8 (WINDSTORM CERTIFICATE) or CERTIFICATE OF COMPLIANCE WILL NOT BE ISSUED UNTIL FULL PAYMENT IS RECEIVED. PLEASE ALLOW A MINIMUM OF 7 TO 10 DAYS, AFTER FULL PAYMENT IS RECEIVED BEFORE THE WINDSTORM CERTIFICATE IS ISSUED BY TDI.   • Start Date
  • Contractor/Builder Info
  • Homeowner Info
  • Project Address
  • **TDI Product Evaluation# (does not apply to comp shingle roofs)
  • Type of Structure 
  • Location of windows/doors (if submitting window or door inspection request)
  • Location of metal patio covers or carports (if submitting roof inspection)
  • Your name & email
